An Overview of IRIS Services in Wisconsin

TMG is one of 4 IRIS consultant agencies in the area and one of the few that is statewide. An IRIS consultant works with individuals with disabilities whether cognitive/intellectual/physical and frail elders to assist them with finding services for individuals to stay in their home, find day services, pay caregivers, find transportation, and many other needs. IRIS is one of the long-term care programs offered by the department of health and human services.
To qualify for IRIS, an individual needs to have:
Medicaid insurance
A social security disability determination
Functional and financial eligibility
If a person meets these qualifications, he or she will then go to their local county’s aging and disability resources center to start the process of signing up for the IRIS program.
The ADRC workers would then:
Conduct the functional screen to determine functional eligibility
Let the individual know his or her choices for an IRIS consultant agency
Inform the individual of their monthly budget that can be used to secure services.Â
The individual would then work with the IRIS consultant to figure out what services the individual needs and assist with securing those services.
