FoodShare Wisconsin

Provides supplemental income to purchase food from a Quest card.
Gross monthly income limit - single $2,610 or couple $3,526
No asset test. Minimum benefit per month is $24
BadgerCare Plus Medicaid

Must be under age 65 and not determined disabled.
Monthly Income Limit single $1,330 or couple $1,803.33
Elderly, Blind or Disabled Medicaid

Must be age 65 or older, Blind, or Disabled
Asset Limit: single $2,000 or couple $3,000
Monthly Income Limit single $1,330 or couple $1,803.33
If applicants income is above the listed amount. Medicaid benefits are still available through
meeting deductible within a 6 month period. The deductible can be met with outstanding
medical bills or paying the deductible directly.
Medicaid Purchase Plan (MAPP)

Full Medicaid program for individuals determined disabled, working a minimum of 1 hour per month or enrolled into a health and employment counseling program.
Asset Limit: $15,000 applicant only.
Monthly Income Limit: single $3,325 or couple $4,508.33
A premium may be required depending on gross monthly income.
Medicare Premium Assistance Programs

QMB (Qualified Medicare Beneficiary)
Monthly Income Limit single $1,330 or couple $1,803.33
QMB pays Medicare Part B premium and Part A and Part B deductibles & coinsurance
SLMB/SLMB+ (Specified Low-Income Medicare Beneficiary)
Monthly Income Limit single $1,795.50 or couple $2,434.50
SLMB/SLMB+ pays Medicare Part B premium
SeniorCare Prescription Assistance

Age 65 or older, Wisconsin Resident, requires $30 annual enrollment fee per person.
$5 co-pay for each generic and $15 co-pay for each brand name prescription after deductible or spenddown is met. If level 2B or 3 may qualify for a Medicare Special Enrollment period to change your Medicare plan.
Level 1 –Annual Income single $25,536 or less or couple: $34,624 or less
No deductible or spenddown.
Level 2a – Annual Income single $25,537 - $31,920 or couple $34,625 - $43,280
$500 deductible per person.
Pay the SeniorCare rate for prescriptions until the $500 deductible is met, then
Level 2b – Annual Income single $31,921 - $38,304 or couple $43,281 - $51,936
$850 deductible per person
Pay the SeniorCare rate for prescriptions until the $850 deductible is met, then
Level 3 – Annual Income single $38,305 or greater or couple $51,937 or greater
Pay retail price for drugs equal to the difference between your income and
$37,561 per individual or $50,761 per couple. This is called a “spenddown.”
After spenddown is met, $850 deductible per person.
Low Income Subsidy (Extra Help)

If receiving Medicaid, automatically approved for benefits.

Energy Assistance (October 2025 – May 2026)

Previous Monthly Income limit - single $3,201.75 or couple $4,186.92
Energy Assistance provides help with electric, gas, fuel and water bills.

Weatherization 920-731-7566 This program can also be requested for an evaluation of your
home energy efficiency. Work performed often includes insulation, sealing air leaks,
installation of energy-saving products, and repair or replacement of inefficient furnaces or
other appliances.
Lifeline Program

Free cell phone service may include data, text and a phone. Benefits vary by service provider
chosen. Applicants qualify if receiving FoodShare, Medicaid, Federal Public Housing
Assistance, Veterans pension and survivors benefit. Applicants can also qualify if household
income is below 135% FPL: single household $1795.50 and couple $2434.50 per month.
Local Hospital Assistance programs to help with unpaid medical costs.
Eligibility varies by plan.
Prescription Drug Assistance programs that work with Medicare to cover your high cost
medications. Eligibility varies by plan.
